Fertility Benefit Enhancement

You asked, and we delivered! At LSS, we are committed to supporting our team members through all aspects of their lives. That’s why we are thrilled to announce significant enhancements to our fertility benefits package, effective January 1, 2024, across all four medical plans.

 Anthem’s infertility benefits will cover all Assisted Reproductive Technologies (ART) and the associated diagnostic testing, up to a lifetime member maximum of $50,000, to cover all treatments including:

• AI (artificial insemination)

• GIFT (gamete intrafallopian transfer)

• ZIFT (zygote intrafallopian transfer)

• IVF (in vitro fertilization) 

We understand the importance of easing the financial burden associated with fertility treatments. The team member’s financial responsibility is based on plan share (i.e., copays, deductibles, coinsurance, etc.) and has a lifetime member maximum benefit of $50,000. Pharmacy claims processed through Express Scripts will not affect the lifetime maximum.
