These programs specifically address joint and muscle health (musculoskeletal or MSK) and chronic conditions such as diabetes and hypertension, providing achievable plans that can be conveniently completed from the comfort of your home and with no costs for our medical plan members.
Omada for Diabetes
Omada for Diabetes is a personalized program with one-on-one coaching, specialist support and smart devices to help members lose weight, manage diabetes, and lower blood pressure. The Omada Diabetes and Hypertension programs can help you to manage diabetes by controlling your blood sugar with less stress and more confidence. It can also help you control blood pressure outside of just medication with dedicated support. If you are eligible, as a member you will be provided a personal health coach, a clinical specialist, all smart devices you need, a personalized care plan, weekly lessons, tools for managing stress and online peer group and communities.
Omada for Hypertension
Omada for Hypertension is a program that offers a better way to manage your blood pressure with a plan designed around you and all the tools and support that you need. The Omada program for Hypertension will help you lose weight, gain energy and lower blood pressure. If you are eligible to become a member of the Omada for Hypertension program, you will receive a personalized care plan, personal support, and better ways to lower your blood pressure.
Omada for Joint & Muscle Health
Omada for Joint & Muscle Health is a virtual physical therapy program that connects you with your very own dedicated licensed physical therapist to diagnose and treat your aches and pains – all from your smartphone or tablet. With your personal Omada physical therapist, you will reduce pain, move better, prevent injuries, and learn to manage stress. If you are eligible to become a member of the Joint & Muscle program, you will receive a dedicated licensed physical therapist, a personalized treatment plan, unlimited video visits, free exercise kit and tools for a healthy mindset.
