If you have LSS Anthem medical insurance, you can get four free over-the-counter at-home COVID-19 testing kits per month, per covered member.  

You can get kits every month at any in-network pharmacy by presenting your LSS medical insurance card and processing the order at the Pharmacy check-out area. Keep in mind, some pharmacies may require an actual prescription for the COVID-19 tests from your provider in order to receive your free tests at the time of the sale or you will need to pay out of pocket and request reimbursement from Express Scripts.
